What is Personal Information and why do we collect it?

Only information that is necessary to provide allied health services and to manage provider/client relationships is collected. The information collected may include you and your child’s: name, date of birth, address, funding details and information about your health history and family history. Information is required to enable services to be delivered.

This Personal Information is obtained in many ways including questionnaires, therapy sessions, interviews, correspondence, by telephone and facsimile, by email, via our website www.littlesunshineot.com.au, from other publicly available sources, and from third parties. We don’t guarantee website links or policy of authorised third parties.

When we collect Personal Information we will, where appropriate and where possible, explain to you why we are collecting the information and how we plan to use it.

Third Parties

Where reasonable and practicable to do so, we will collect your Personal Information only from you. However, in some circumstances we may be provided with information by third parties. In such a case we will take reasonable steps to ensure that you are made aware of the information provided to us by the third party.

Use and Disclosure of Personal Information

Little Sunshine Occupational Therapy uses you and your child’s personal information for the purpose of providing clinical therapeutic services. Information will be used to inform direct service delivery, to manage therapist/client relationship and to contact you in relation to matters concerning your child’s supports. Information may also be used for any other purposes permitted under the Privacy Act 1988.

We will ensure that all personal information gathered by us remains confidential and secure except when:

1. Required or authorised by law or 
2. Failure to disclose the information would put you or another person at risk; or 
3. Your prior approval has been obtained to release written material or discuss the material with a third party eg. Paediatrician, therapist or school

Securing your Personal Information

Your Personal Information is stored in a confidential and secure manner that reasonably protects it from misuse and loss and from unauthorised access, modification or disclosure.

When your Personal Information is no longer needed for the purpose for which it was obtained, we will take reasonable steps to destroy or permanently de-identify your Personal Information. However, most of the Personal Information is or will be stored in client files which will be kept by us for a minimum of 7 years.

Access to your Personal Information

You have a right to access the personal and health information that we hold about you and your child and to update and/or correct it, subject to certain exceptions. If you wish to access your Personal Information, please contact us in writing.

In accordance with the Australian Privacy Principals, Little Sunshine Occupational Therapy may deny access to you or your child’s clinical records, should it be deemed that giving access would pose a serious threat to the life, health or safety of any individual, or to public health or safety.

Little Sunshine Occupational Therapy will not charge any fee for your access request, but may charge an administrative fee for providing a copy of your Personal Information.

In order to protect your Personal Information we may require identification from you before releasing the requested information.

It is an important to us that your Personal Information is up to date.
